Complete the Application and submit it to us via, mail or drop off at the Front Office. Once the Application is received, the enrollment process will begin.
Once you have secured a spot with FSCP:
Two dates in May are reserved for families to come in to complete the paperwork. Forms must be completed prior to their first day of attendance. The Agreement must be completed in the Front Office.
To save time, the forms below can be completed and brought with you. The forms are printable PDF files. Click on each to download the form to your computer, complete, print, and submit to the Front Office. Please be sure that all information on the forms are completed. Forms must be signed in the office. **Note: Forms will not be accepted via email

SIGN): *Emergency Contact, *Emergency Evacuation, *Parent Release, *Handbook Acknowledgment, *Sunscreen Permission, *Babysitting Release, *Health Assessment, *Covid-19 Risk and Waiver and *Family Acknowledgement.
If you have more than one child enrolled at Four Seasons, you may want to copy the forms to make updating information easier in the future.
The state paperwork process is done yearly.
